Approaching Retirement & Medicare Enrollment
When you turn 65, you'll become eligible for Medicare. Since you're still working and covered by our company health plan, you're not required to switch, but you do have the option to enroll in Medicare.
If you choose to apply for Medicare Part B while remaining on our plan, you'll need to complete Form CMS-L564 (Request for Employment Information) to verify your coverage. You should fill out Section A with your personal details, and HR will complete Section B. Once completed, submit the form to Social Security along with your Medicare Part B application (Form CMS-40B).
While we cannot provide formal Medicare advice, you generally have three options once you become eligible:
Remain on the company-sponsored group health plan
Move fully to Medicare
Combine both plans (with Medicare as either primary or secondary coverage)
